Definition

The AI accountability problem is structural: AI systems are typically developed by one organization, deployed by another, configured by a third, and used by a fourth — with each entity able to shift responsibility to the others when harm occurs. Terms of service disclaim liability, developers point to deployers, deployers point to users, and affected individuals find no entity willing to accept responsibility.

This factor frequently co-occurs with regulatory gap (CAUSE-013) and over-automation (CAUSE-010), confirming that the absence of assigned responsibility is most harmful when regulation does not impose it and automated systems make decisions without human accountability.

Why This Factor Matters

Accountability vacuums have enabled sustained, systematic harm because no entity accepts responsibility for correcting the problem. The Australian Robodebt scheme (INC-16-0001) sent unlawful debt notices to hundreds of thousands of welfare recipients over several years. When individual cases were challenged, government agencies pointed to the automated system; when the system was questioned, agencies pointed to policy decisions; when policy was questioned, responsibility diffused across multiple government departments. A royal commission was ultimately required to assign accountability.

The Boeing 737 MAX MCAS failures (INC-18-0003) killed 346 people in two crashes. The accountability chain — Boeing as developer, airlines as deployers, pilots as operators, regulators as overseers — initially obscured where responsibility lay for the MCAS system’s design and the decision to deploy it without adequate pilot training.

The Uber autonomous vehicle fatality (INC-18-0001) raised the question of accountability for autonomous system decisions: is the developer liable for the system’s failure to detect a pedestrian, the safety driver for failing to intervene, or the organization for deploying the system with inadequate safety protocols?
